Remote Rehabilitation Specialist
Job Details
The Rehab Specialist has the responsibility of providing education and training to adults with serious mental illness aimed at developing problem-solving skills, enhancing self-esteem, developing pre-vocational skills, improving social/relationship skills, and facilitating enhanced community integration and recovery within a psychosocial rehabilitation setting.
- Complete biopsychosocial/functional assessments and develop individualized rehabilitation plans for adults and minors who suffer from a variety of mental health challenges.
- Provide psycho-educational activities and conduct groups to teach persons served about mental health, wellness, self-help, and appropriate medication to avoid complications and relapse.
- Provides supervision to direct care staff by conducting individual and /or groups supervision to Direct Service Workers.
- Communicates with Behavioral Health colleagues, Primary Care providers, therapists and other providers and agencies as needed to facilitate integrative care.
- Monitor weekly/monthly notes, weekly/monthly collaboration notes, contact notes, client visit logs for any on-site programming.
